Form DS-10 (Birth Affidavit)

The Form DS-10, Birth Affidavit, is an important document for those who were born outside of the United States and need to establish proof of U.S. citizenship. The form must be filed by the applicant's parent or guardian, and it must be accompanied by the applicant's birth certificate and proof of the parent or guardian's U.S. citizenship. Once approved, the Form DS-10 will be used to obtain a passport or other travel document. While it may seem like a daunting task, filing the Form DS-10 is a relatively straightforward process. In most cases, it can be completed within a few weeks.


The Birth Affidavit (DS-10) is used to request a certification of birth if you do not have a certified copy of your birth certificate. The Affidavit must be signed by you and by someone who has personal knowledge of your birth. The Affidavit must be notarized by a Notary Public before it can be submitted. You will also need to submit proof of identities, such as a driver's license or passport, and pay a fee. Once the Affidavit is processed, you will receive a certified copy of your birth certificate.
