Form DS-1350

Updated: Jun 14, 2022

Form DS-1350 was a document used to establish the US citizenship of individuals born overseas. The form was discontinued in 2010 and is no longer valid for establishing citizenship. However, if you were born overseas and have a Form DS-1350, it may still be helpful to submit it as part of your application for US citizenship. The form can help to establish your identity and prove your US citizenship. If you are submitting Form DS-1350 as part of your citizenship application, make sure to include any supporting documentation, such as a birth certificate or passport. For more information on applying for US citizenship, contact the US Citizenship and Immigration Services (USCIS).
