Form DS-5507

The DS-5507 form is used by a U.S. citizen or non-citizen national to establish paternity/ support for a child born out of wedlock. The father signs the form to confirm they are the child's biological parent and agree to provide financial support. Once the form is signed, it can be used as legal proof of paternity in court proceedings or to help the child obtain government benefits such as social security or veteran's benefits. The form must be signed by both the father and child's mother in order for it to be valid - if the mother does not sign, the father can still establish paternity through DNA testing. A DS-5507 form is an important tool for ensuring that children born out of wedlock have access to the financial support they need to thrive.
