Form DS-11

The DS-11 form is the standard application used to apply for a U.S. passport. It must be completed in full and submitted in person at a passport acceptance facility, along with proof of U.S. citizenship and a valid form of identification. The form asks for basic biographical information, as well as your travel plans and contact information. Once the form is complete, you will need to have your photograph taken and submit any required supporting documents. If everything is in order, you should receive your passport within six to eight weeks.
